No the undershirt is not included, It was to difficult at the scale, to go through with the undershirt illusion. He must've already had it.

Nope, it was a difficult task for the tailor, he could have included fake sleeves, but the inner part was not easy to be sewn in, also the threads would have been visible, so the whole idea was scratched.

I just got the white undershirt off ebay, is was only cheap $4.99 with free delivery.
